Diagnosis, Assessment and Documentation

To be approved for ongoing services, students should upload appropriate documentation in the AIM system or send documentation to Disability & Accessibility Services (DAS) at das@georgefox.edu .

Such documentation might include a completed DAS documentation form, medical documentation, individualized educational plans (IEP), 504 plans, cognitive or psychoeducational assessments, or other relevant documents. Contact us if you have questions about documentation.

The last two items are of particular value in determining academic accommodations and adjustments. All documentation is reviewed on an individual basis.

Diagnosis/Assessment for Students

Costs associated with diagnosis, evaluation, and assessment or reassessment generally are the responsibility of the student. For students currently enrolled at George Fox, information and referral to the student Health and Counseling Center and/or other professionals in the community may be appropriate. It is important to recognize that accommodation needs can change over time and are not always identified through the initial diagnostic process. Conversely, a prior history of accommodation does not, in and of itself, warrant the provision of a similar accommodation.

The George Fox University Behavioral Health Center provides low-cost psychological services to the community, including psychoeducational assessment.
