Upload Documentation
Meal PlanHousing/Meal Plan Accommodation Provider Form
If your provider prefers to write a letter, the document should include the disability/medical/mental health diagnosis and detailed accommodation recommendations. Those recommendations should focus on necessary environmental or dietary needs, not specific locations. For example, a provider might indicate you need a location where you can access a kitchen, not that you need housing in specific campus apartments. NOTE: We cannot make a determination until we have documentation from an appropriate provider.
